RULES & REGULATIONS FOR FELLOWSHIP

Academic Head: Dr. S. Natarajan, Chairman & Medical Director
The Schedule is as follows:
Monday / Wednesday / Friday: OPD days
Tuesday / Thursday / Saturday: OT days
In Rotation
1st Fellow: Diagnostics
2nd Fellow: Diagnostics/ OPD
3rd Fellow: OPD

I] General Rules
  • You have to report to Hospital at 8.00 am sharp till late evening daily.
  • Fellowship stipend will be Rs. 8000/- per month.
  • NO accommodation will be provided to you.
  • Have to submit the joining letter on first day
  • You will have two other accompanying fellow doctors.
  • Have to bring your White Coat and wear it during your duty hours in hospital premises.
  • Have to bring your 20D & 78D or 90D lens and also Scleral Depressor.
  • Available on telephone for emergency situation.
  • Enter patients' medical records onto the computer and prepare thanks letter for doctors, case     summary, etc.
  • The training being intensive, you are advised not to take leave unnecessarily for wedding,     birthday, engagement parties etc. Total leave allowed: 15 days
  • If you require leave for unforeseen circumstances, you will take prior sanction from Medical     Director as well as from Managing Director.
  • To speak with all the patients politely, not to get involved in the arguments and unnecessary     loose talk with the patients.
  • Doctors are requested to keep their belongings in the proper place provided to them.
  • Have to manage patients in his absence.
  • Clarifications if any, to be confirmed with Dr. S. Natarajan / other consultants.

    II] OPD
  • You will be responsible for seeing all Dr. Natarajan's / other consultant's patients, before they     consult with them on your name.
  • Whenever possible, advise the necessary investigations, so that when they reach Dr.     Natarajan / other consultant, they will be fully worked up, so that they can give their opinion.
  • When in doubt, ask Dr. S. Natarajan / other consultants for advice.
  • Take care to see that handicapped patients, very old patients, SAPs, debilitated patients are     seen quickly without much shifting & delay.
  • Prior to sending the patients to Dr. Natarajan / other consultant, the patients should be fully     worked up for other cataract, glaucoma, opinions if neeeded.
  • Write your findings, diagnosis, & investigations, but do not communicate your opinion to the     patient, Dr. S. Natarajan / other consultant will be advising them on the course of Rx.
  • Utilizes the rest of the time for project work.

    III] Diagnostics
  • Fellow in charge of diagnostics is fully responsible for the patient.
  • You are required to be standby for FFA. You should check the FFA files and accordingly     supervise Medical Photographer.
  • You must see that the pre - angiography consent forms and that the evaluation forms are     signed.
  • Oxygen cylinder must be checked once a week. The emergency tray must be checked once a     month & must be kept within accessible reach.
  • In case of patients who are allergic to certain drugs & eyedrops. It is necessary to call the     allergy specialist as a standby.
  • In an event of any type of reaction, the emergency management protocol is pinned up on the     wall which is to be followed.
  • Diabetics (especially brittle diabetics, patients with nephropathy etc.), heart patients,     clearance from the physician is necessary and always remember to keep 25% dextrose as a     standby in c/o hypoglycemic attacks.
  • In cardiac patients anesthetist will assist you, however the overall responsibility of the     diagnostics room is yours.
  • In case any of the equipment require servicing or any problem arising in the management of     patient inform Dr. S. Natarajan / other consultant & contact the concerned servicing agents     and fill up the complaint register.
  • You will be required to fill up the investigation register each investigation à complete the     records. This is especially important to recover data, while making for scientific presentations     and publication. This register will make your job easier.
  • When in doubt about the working of any instruments be sure to ask the consultants.
  • Reports must be sent to the respective consultants as soon as they are ready.
  • In case of any mishap in the diagnostics room, immediately inform Dr. S. Natarajan / other     consultant.
  • Can utilize the rest of the time for project work.

    IV] OT
  • Fellow should be in OT 30 mins. prior to the starting of the OT.
  • Work up patients who are getting admitted for surgery.
  • Check their physical fitness, investigations to be done before surgery as well as prepare     retinal drawings. Patients may get admitted late in the evening but still pre-operative detailed     retinal drawings should be done with international colour coding properly.
  • Pre-operatively you should explain to the patients the pros and cons about surgery and clear     all their doubts.
  • You must inform Dr. S. Natarajan / other consultant in case of any delays morning     investigations ordered for patients on the day of surgery.
  • You must be in OT, at the time of shifting of the patient, blocking (or GA) and must be     washed up before Dr. S. Natarajan / any consultant is in the OT.
  • You must check that all instruments, autoclaving and equipment and sign in the     corresponding register.
  • Responsible for assisting in surgery, as instructed by Dr. S. Natarajan / consultants.
  • After the surgery, you are responsible for completing post operative orders and registers.
  • You are responsible for seeing the post operative on the following day before Dr. S.     Natarajan sees his patients. Post operative examination should include all details.
  • Post - operative dressing to be done and reported to the Surgeon.
  • Ensure that before discharge all formalities are completed. Make sure that Discharge     Summary is made ready by the Secretary and initial it.
  • You should complete all medical records including admission records, operation notes,     postoperative orders, etc.
  • You should also see all the set protocols are followed by nursing staff in Ward and O.T.
  • On OT days, it is the collective responsibility of all the fellows to see that preoperative     diagrams and preoperative photos are taken, especially the fellow on duty in the OT the next     day.
  • The files (Inv, consent, recent findings etc.) should be checked 1 day prior.
  • The timings & order of the operations should be discussed with Dr. S. Natarajan one day     prior.

    V] Diagnostics / OPD
  • Fellow will be required to attend OPD & OT if there is any case in OT.
  • B- Scans will be done by this fellow.
  • Accommodates the duty of the absent fellow.

    VI] Academic:

    Classes
  • Morning classes are conducted on Monday and Friday at 8. 30 am sharp for 1 hour.
  • To attend the classes and duty on time.
  • Fellows will be given their topics 1 month in advance. And in the case of journal clubs they     will receive the topics 1 week in advance.

    Log Book
  • Log Book to be maintained and monitored weekly by the consultants.

    Library
  • All the Doctors are requested to maintain silence in the Library.
  • All are requested to sign the Library Register Book while taking and returning the book.
  • No books will be allowed to be taken out of hospital premises.
  • Books will be issued between 11.00 am to 7.00 pm.
  • Books should be returned before 7.00 pm
  • If anyone requires books from the library for any preparation for Dr. S. Natarajan's     presentation or for any other purpose, please plan in advance and issue well in time.
  • All are requested to handle the books with care and not to leave them anywhere in the     hospital premises.
  • All are requested to return the books within 15 days from the date of issue.
  • Kindly see that the cupboards are locked properly after taking the book.
  • If "case file" for some reason are brought to the Library, they should be returned to their     respective place, same time by the person concerned before you leave the library.
  • No meetings with the clients or medical representative in the library are allowed. Kindly take     them to the waiting hall.
  • No personal discussions & chit chatting.
  • Only one book at a time can be taken.
  • No Lunch should be taken in library under any circumstances.
  • Also, the CDs from the CD Library are not allowed to take outside the hospital premises. It     has to be handled carefully and returned back without any damage to the Librarian.
  • You have no permission to copy the data from the CD for your personal records.

    Project work
  • All fellows are responsible for preparing Dr. S. Natarajan's presentations both Power Point     slides and text for various conferences. In addition to this, they are also responsible to help     other Consultants with their Academic work.
  • Fellows should complete their project work, articles and send for publication.
  • Project work will be reviewed periodically.
  • To complete the allotted work like Research papers and presentations on time. No last minute     hassle.

    Computer Facility
  • Consultant Computer is provided with the facility of 24 hrs. Internet, CD-Rom, Speakers, &     Scanner.
  • All fellows have to strictly work on the Consultants Computer Only & not to use the Academic     Computer.
  • In case they are using the Academic Computer they will be responsible for any loss of data     being saved on Academic Computer.

    On calls
  • One fellow will be required to stay on campus every Sunday from 9.30 am to Monday     Evening.
  • The duty roster will be prepared along with a consultant on call.
  • In the event of any emergency, the fellow on call must inform the concerned consultant / Dr.     S. Natarajan.

    VII) Charitable Camps
  • You will be required to go for camps organized by the Aditya Jyot charitable wings (Sion,     Dadar & Mahalakshmi). This is done in rotation and will be informed accordingly.
  • Everyday you will man the free Vitreo Retinal Clinic.

    VIII) Lunch Table Manners
  • NO lunch time as such and are expected to go only on rotations for a short break.
  • Lunch should be taken only in the place allotted (4th Floor - Terrace) and nowhere else.
  • NO permission will be given to have lunch after 4.00 pm.
  • Not more than 2 persons will have their lunch at a time.
  • Kindly maintain cleanliness and silence while eating.
  • Please ensure the newspapers are spread on the table before eating and thrown in the bin     after eating.
